Indemnity and Consent Agreement

By checking the box and submitting this form, I, the parent/legal guardian of the registered child/children, hereby agree to the following:

1. Consent for Participation

I consent to my child’s/children's participation in the Blast Youth Camp (hosted by Father’s Heart Digital Church) from 27 September – 1 October 2026 at Delsma Farm Venue. This consent extends to all camp activities and any organized transport required during the event.

2. Indemnity and Risk

I understand and accept that participation in camp activities is at my child’s own risk. On behalf of myself, my executors, and my child, I hereby indemnify, hold blameless, and absolve Father’s Heart Digital Church, its staff, and any authorized volunteers against any claims for injury, illness, or loss of/damage to property occurring during the event. I provide this indemnity with the understanding that the leadership team will take all reasonable precautions for the safety and welfare of the participants.

3. Medical Authority (In Loco Parentis)

I authorize the camp leaders to act "In Loco Parentis" (in place of a parent) during the event. In the event of an accident or illness requiring medical attention in the opinion of the leadership, I accept full responsibility for any medical costs incurred.

4. Media Consent

I grant consent for the use of appropriate photographs or video footage of my child for reporting and promotional purposes, provided my child also consents to being photographed at the time.
